You can schedule an appointment by clicking here. On the scheduling page, select the provider and appointment of your preference, then select the date and time from the calendar prompt. Fill in your information and book your appointment. You will then receive a confirmation email at the address you provided, along with prompts to finish preparing for your appointment, such as completing intake forms.

The clinic is located in Wallingford right across the street from Gasworks Park, at 3301 Burke Ave N, Suite 360. This is the building on the southwest corner of Burke Ave N and N Northlake Way, set uphill from Northlake Way and accessed from Burke Ave. There is plenty of free street parking, and you can also park for free in the Gasworks parking lot if necessary. Please do not park in the building lot as those spots are reserved for the tenants. 

When you arrive the front entrance is located on the uphill side of the building. You will need to either walk up through the parking lot, or you can come around the back of the building by cutting through the alley from Wallingford, one block to the west. The building is not wheelchair accessible, and the clinic is located up one flight of stairs after you enter the building. No door code is required, and the front door is unlocked during the day. If the door is locked unexpectedly please call or text and someone will come down to open the door for you.

When driving to the clinic be aware of the intersection at Fremont and 34th Ave N, as you cannot turn left onto 34th driving south on Fremont. If you are coming from the northwest you will need to turn left onto 35th Ave to avoid going over the Fremont bridge.

If you have a telehealth visit scheduled you will receive a link in your confirmation and reminder messages to start a video visit at the scheduled time. You will also receive an additional reminder with the link if you are not logged in when your visit begins.
